Stat. § 81-6,102","heading":"Diagnosis; report; contents.","body":"(1) If a resident of this state is diagnosed with Parkinson's disease or a related movement disorder within this state in the office of a physician licensed under the Uniform Credentialing Act, the physician shall file a report of the diagnosis and pertinent information with the department within sixty days after the diagnosis.\n(2) An individual resident of this state who has been diagnosed with Parkinson's disease or a related movement disorder by a licensed physician may file a report with the department providing relevant information. The department shall provide for validation of individual reports.\n(3) A report filed under this section shall contain the following information about the person diagnosed with Parkinson's disease or a related movement disorder:\n(a) Name;\n(b) Social security number;\n(c) Date of birth;\n(d) Gender;\n(e) Address at time of diagnosis;\n(f) Current address;\n(g) Date of diagnosis;\n(h) Physician;\n(i) Identification of reporting source; and\n(j) Any additional information the department demonstrates is reasonable to implement the Parkinson's Disease Registry Act.","path":["Chapter 81 - STATE ADMINISTRATIVE DEPARTMENTS"],"source_url":"https://nebraskalegislature.gov/laws/statutes.php?statute=81-6,102","current_through":"Laws 2026","vintage":"","retrieved_at":"2026-09-17T21:01:11Z","sha256":"1adcaa3b9d32a982d996ca7ea66c4b5ae5d91b7112a6e757c1ecbcdd86a0176b","source_id":"us-ne","stale":false,"prev":"us-ne/neb.-rev.-stat.-81-6-101","next":"us-ne/neb.-rev.-stat.-81-6-103"},"notice":"GroundRules: Original legal text.
