The application for a commercial driver license or commercial driver learner permit shall include the following:\n(a) The full name and current mailing and residential address of the license applicant;\n(b) A physical description of the applicant, including gender indicated by checking either \"M\" for \"male,\" \"F\" for \"female,\" or \"X\" for \"other,\" height, weight, eye and hair color;\n(c) Date of birth;\n(d) Applicant's social security number, unless the application is made by the resident of a foreign jurisdiction for a commercial driver license;\n(e) Applicant's signature;\n(f) Certifications of driver qualification;\n(g) Any other information required by the department.\nII. The application shall be accompanied by the fee provided in RSA 263:42.\nIII. When a licensee changes his name, mailing address, or residence, he shall notify the department as provided in RSA 263:9.\nIV.
