Applications for original certification, renewal license or certification, and examinations shall be made in writing to the board on forms approved by the executive director of the office of professional licensure and certification.\nII. Appropriate fees, as established by the executive director of the office of professional licensure and certification, shall accompany all applications for original license, certification, renewal license, renewal certification, reciprocal license, and reciprocal certification.\nIII.
