In response to board inquiries relevant to a licensee's status or practice of nursing or nursing-related activities, each licensee shall provide complete and truthful information.\nII. Each licensee shall notify the board if a license is lost or stolen.\nIII. Each licensee shall notify the board of a change of name or address.\nIV.
