All licensed pharmacy practice sites shall report to the board the occurrences of any of the following:\n(1)\tClosing of the pharmacy practice site;\n(2)\tChange of ownership, location, interior site design, permit classification or pharmacist-in-charge of the pharmacy practice site;\n(3)\tAny significant theft or loss of legend drugs or devices;\n(4)\tDisasters, accidents, any theft, destruction or loss of records required to be maintained by State or federal law;\n(5)\tAny pharmacy malpractice liability insurance claim settlement, judgment or arbitration award in excess of $10,000 to which an owner, an employee of, or the pharmacy practice site itself is a party; and\n(6)\tAny and all other matters and occurrences as the board may require by rule or regulation.\nb.\tThe manner, time and content of the notification shall be prescribed by rule or regulation by the board.\nL.2003,c.280,s.35.","path":["TITLE 45 PROFESSIONS AND OCCUPATIONS"],"source_url":"https://pub.njleg.state.nj.us/statutes/STATUTES-TEXT.zip","current_through":"P.L.2025, c.405, and J.R.22","vintage":"","retrieved_at":"2026-08-27T17:54:13Z","sha256":"884de7cfc18ad752f5a79fdaf75c8bde72d566900eeab86ca7c8142be1163d93","source_id":"us-nj","stale":true,"prev":"us-nj/n.j.-stat.-45-14-73","next":"us-nj/n.j.-stat.-45-14-75"},"notice":"GroundRules: Original legal text.
