Stat. § 52:27H-22.11","heading":"Fees","body":"5. \tThe following fees shall be assessed, in an amount to be determined by the commissioner, in connection with the voluntary certification provided for under this act:  a.  An initial application fee not to exceed $250; b.  An annual renewal fee; and c.  A reinstatement fee.
