The clerk of a court entering a final decree or order in an adoptive placement of an Indian child shall provide the secretary with a copy of that decree, adoptive placement order and any other information necessary to show:\n(1) the birth name and birthdate of the Indian child;\n(2) any information relating to tribal membership or eligibility for membership of the adopted Indian child;\n(3) the tribal affiliation and name of the Indian child after adoption;\n(4) the names and addresses of the biological parents;\n(5) the names and addresses of the adoptive parents;\n(6) the name and contact information of any agency having files or information relating to the adoption; and\n(7) any affidavit signed by the biological parent or parents asking that their identity remain confidential.\nB.
