The application for a commercial driver's license or commercial driver's instruction permit shall include the following:\n(1) the full name and current mailing and residential address of the person;\n(2) a physical description of the person, including sex, height, weight and eye color;\n(3) the person's date of birth;\n(4) the person's social security number;\n(5) the person's signature;\n(6) a consent to release the person's driving record information;\n(7) certification by the applicant that the commercial motor vehicle used for the knowledge and skills test for driving a motor vehicle is in the class of commercial motor vehicles for which the person has applied for a commercial motor vehicle license;\n(8) certification by the applicant that the commercial motor vehicle used for the knowledge and skills test for driving a motor vehicle is representative of the endorsement for which the person has applied; and\n(9) any other information required by the department.\nB.
