After an order of confirmatory adoption or readoption has been entered, the court shall direct the petitioner or the attorney of the petitioner to prepare a report of adoption on a form prescribed and furnished by the State Registrar of Vital Statistics or the equivalent office in the state that issued the birth certificate. The report may:\n(a) Identify the original certificate of birth of the person adopted;\n(b) Provide sufficient information to prepare a new certificate of birth for the person adopted;\n(c) Identify the order of adoption; and\n(d) Be certified by the clerk of the court.\n2.
