The application shall include:\n1. The name and address of the applicant or applicants.\n2. The legal description of the place where the proposed assembly is to be held.\n3. The date or dates of the assembly.\n4. The estimated attendance at the assembly.\n5. The nature or purpose of the assembly.\n6.
