The name, address and contact information of the scholarship organization;\n2. The total number of such donations, gifts and grants received by the scholarship organization during the immediately preceding calendar year;\n3. The total dollar amount of such donations, gifts and grants received during the immediately preceding calendar year;\n4. The total number of pupils for whom the scholarship organization made grants during the immediately preceding calendar year pursuant to NRS 388D.270;\n5. The total dollar amount of such grants made during the immediately preceding calendar year; and\n6.
