The name, address, contact information, position held on the governing body and any other information required by the licensing authority of each member of the governing body;\n2. A copy of the articles of incorporation, constitution and bylaws of the foster care agency;\n3. Evidence satisfactory to the licensing authority that the foster care agency has the ability to financially support and sustain its activities, which may include, without limitation, financial statements and budgets;\n4. A report from an independent auditor of the complete financial information for the foster care agency for the immediately preceding fiscal year;\n5. A statement of purpose; and\n6.
