A person, state or local government unit or agency thereof that wishes to operate or operates a small child care establishment may register the small child care establishment with the Division by submitting to the Division on the Internet website of the Division the following information:\n(a) The name, address and contact information of the operator of the small child care establishment;\n(b) The name and address of the small child care establishment;\n(c) An affirmation that the operator of the small child care establishment is in compliance with subsection 2; and\n(d) Such additional information as the Division deems necessary.\n2.
