The Department may require the holder of a general permit to apply for and obtain an individual permit.\n2. An individual permit may be required by the Department in any of the following cases, without limitation:\n(a) If the discharge or injection of fluids through a well is not in compliance with the conditions of the general permit.\n(b) If a change in circumstances has occurred and the general permit is no longer applicable.\n(c) If the Department determines that the discharge or injection of fluids is a significant or potentially significant contributor of pollutants.
