The operator of a crematory, funeral establishment or direct cremation facility shall keep a record of:\n(a) Each authorization received;\n(b) The name of each person whose human remains are received;\n(c) The date and time of receipt, and a description of the container in which received;\n(d) The date of cremation; and\n(e) The final disposition of the cremated remains.\n2. The operator of a crematory shall not accept unidentified human remains. If the remains are received in a container, the operator shall place appropriate identification upon the exterior of the container.\n3.
