The name of the inspector;\n2. The kind and nature of service rendered;\n3. The particular locality where the work was done;\n4. The length of time employed;\n5. The number of sheep inspected and the name of the owner or person in charge of the sheep;\n6. The disease or diseases treated, and the length of time of the treatment; and\n7.
