A written application for a license to conduct a private employment agency in this State must be made to the Labor Commissioner and must contain:\n(a) The name and address of the applicant;\n(b) The street and number of the building or place where the business is to be conducted; and\n(c) The business or occupation in which the applicant was engaged for at least 2 years immediately preceding the date of the application.\n2.
