The application must be accompanied by:\n1. A financial statement prepared by a certified public accountant or a public accountant, the accuracy of which is sworn to under oath before a notary public by the proprietor, a general partner or a corporate officer or a member authorized to execute such documents;\n2. Any other information requested by the Commissioner or otherwise required in connection with the evaluation and investigation of the applicant’s qualifications and suitability for licensure;\n3. A nonrefundable license fee of $1,000;\n4. A nonrefundable investigation fee of $800; and\n5.
