Each licensee shall maintain a record that includes for each employee:\n(a) The employee’s full name;\n(b) The address of each place at which the employee has resided during the previous 10 years;\n(c) The name and address of each employer during the previous 10 years;\n(d) A recent photograph of the employee measuring 3 by 5 inches; and\n(e) Any alias used by the employee.\n2. A licensee may comply with the provisions of subsection 1 by maintaining the employee identification information required pursuant to 26 C.F.R. Part 31.\n3.
