A health maintenance organization that has been issued a certificate of authority pursuant to chapter 695C of NRS;\n4. A multiple employer welfare arrangement as defined in 29 U.S.C. § 1002;\n5. An employer for whom a program of self-insurance is administered by an administrator; and\n6.
