Procured a certificate of authority from the Commissioner for the establishment, maintenance and operation of the plan.\n2. Paid to the Commissioner the fees required of insurers by NRS 680B.010 for:\n(a) The filing of the initial application;\n(b) The issuance of the certificate of authority;\n(c) Each annual continuation of the certificate of authority; and\n(d) The filing of each annual report.\n3.
