Code § 28-401.5","heading":"§ 28-401.5 Application and conditions.","body":"Every application for a license or certificate of competence shall be made in such form and shall be accompanied by such information as the commissioner may prescribe, and by the required fee. Failure to provide all requested documents in a timely manner will constitute an incomplete application and will result in denial of the license. It is a condition of the license or certificate of competence that information in the application be kept correct and current.
