Code § 5164.302","heading":"","body":"(A) Before entering into a provider agreement with a medicaid provider that seeks initial enrollment as a provider of home and community-based services under the medicaid program, the department of medicaid shall conduct an in-person review of the individual or site inspection of the entity seeking enrollment as a provider. The department shall thereafter conduct a subsequent in-person review or site inspection every three years.\n(B) The department shall deny, refuse to revalidate, suspend, or terminate a provider agreement if the department determines that an individual or entity seeking enrollment as a provider of home and community-based services under the medicaid program is principally located at the same address as more than six other active home and community-based services medicaid providers or is principally located at the same address as another home and community-based services medicaid provider when the address contains less than one thousand square feet of space.\n(C) The department of medicaid shall make a referral to the auditor of state whenever it is determined that a single address is the principal place of business for more than six home and community-based services medicaid providers.","path":["Title 51 Public Welfare","Chapter 5164 Medicaid State Plan Services"],"source_url":"https://codes.ohio.gov/ohio-revised-code/section-5164.302","current_through":"2026-10-06 (Senate Bill 315 - 136th General Assembly)","vintage":"","retrieved_at":"2026-09-14T19:24:40Z","sha256":"d0451c768d81e4d2079e8d7258aa79673544087de48da23a6ccfb8a9bba18577","source_id":"us-oh","stale":false,"prev":"us-oh/ohio-rev.-code-5164.301","next":"us-oh/ohio-rev.-code-5164.303"},"notice":"GroundRules: Original legal text.
