Stat. tit. 10, § 10-1414.1","heading":"Greer Center Facility - Admission guidelines and","body":"procedures.\n\nA. In addition to the admissions requirements of Section 1414\n\nof this title, the Greer Center Facility located on the grounds of\n\nthe Northern Oklahoma Resource Center of Enid in Enid, Oklahoma,\n\nshall be established as a separate entity from the Northern Oklahoma\n\nResource Center of Enid and further shall provide for the admission\n\nof persons who have been dually diagnosed as follows:\n\n1. Primary diagnosis of an intellectual disability by a\n\npsychologist, physician or psychiatrist. The diagnosis shall be in\n\naccordance with any statutory requirements and shall include\n\nintellectual evaluation, adaptive behavior evaluation, and evidence\n\nthat the disability occurred within the developmental period.\n\nPreference shall be given for those individuals whose disability\n\nlevel falls within the mild and moderate ranges; and\n\n2. Secondarily, clinical evidence of behavioral or emotional\n\nproblems pursuant to a formal, written evaluation by a psychologist,\n\npsychiatrist or physician describing the nature of the problem, the\n\nfrequency of occurrence of the problem, any prior treatment efforts\n\nand reasons why the applicant cannot receive appropriate treatment\n\nin the applicant's current environment and a secondary diagnosis of\n\nmental illness in accordance with the Diagnostic and Statistical\n\nManual of Mental Disorders, as revised and published by the American\n\nPsychiatric Association.\n\nB. A person shall not be considered for voluntary admission\n\ninto the Greer Center Facility unless it can be clinically\n\ndemonstrated that the behavior of the person does not pose an\n\nunreasonable risk of injury, death or sexual assault to others or an\n\nunreasonable risk of injury or death to self. Persons considered\n\nfor admission shall not be considered by a psychologist,\n\npsychiatrist, or physician as homicidal or suicidal and shall not\n\nhave exhibited homicidal or suicidal tendencies for six (6) months\n\nprior to application for admission.\n\nC. An applicant who requires skilled nursing care shall not be\n\nadmitted to the Greer Center Facility. Applicants having a medical\n\ncondition which is degenerative in nature that will require skilled\n\nnursing shall be considered on a case-by-case basis to ensure that\n\nsufficient staff is available to ensure quality of care. If an\n\napplicant has any existing medical or surgical condition that is\n\ncorrectable, the condition shall be remedied by the referring\n\nfacility before admission to the Greer Center Facility is\n\nconsidered.\n\nD. Any person seeking admission to the Greer Center Facility\n\nfor treatment who qualifies under subsection A of this section,\n\nsubject to the availability of space, shall be admitted. All\n\npersons admitted to the Greer Center Facility shall submit to the\n\ndirector of the Greer Center Facility a referral packet that\n\ncontains at a minimum, the following information or records:\n\n1. Results of a current physical exam;\n\n2. Recent physician orders and progress notes for up to one (1)\n\nyear, if available;\n\n3. Recent nursing notes for up to one (1) year, if available;\n\n4. Fact sheet (medical records);\n\n5. Legal papers, including, but not limited to, birth\n\ncertificate, marriage certificate and guardianship;\n\n6. Social history, with a recent social evaluation or update\n\nwithin one (1) year;\n\n7. Psychological exam administered or updated within ninety\ngress notes for up to one (1)\n\nyear, if available;\n\n3. Recent nursing notes for up to one (1) year, if available;\n\n4. Fact sheet (medical records);\n\n5. Legal papers, including, but not limited to, birth\n\ncertificate, marriage certificate and guardianship;\n\n6. Social history, with a recent social evaluation or update\n\nwithin one (1) year;\n\n7. Psychological exam administered or updated within ninety\n\n(90) days of referral;\n\n8. Dental records;\n\n9. Immunization record;\n\n10. Multidisciplinary progress notes for up to one (1) year, if\n\navailable;\n\n11. Medical and medication history; and\n\n12. Individual Habilitation Plan or Care Plan, if available.\n\nE. The Greer Center Admissions Committee shall consist of a\n\nrepresentative from the Greer Center Facility, and other\n\nrepresentatives selected by the Director of the Department of Human\n\nServices. The Committee shall make decisions regarding admissions\n\nto the programs of the Greer Center Facility. The Committee may\n\nrequest additional information concerning an applicant from the\n\nreferring agency or participation by referring agency personnel as\n\nnecessary.\n\nF. Persons entering the Greer Center Facility shall receive a\n\ncomprehensive evaluation of their intellectual functioning, adaptive\n\nbehavior skills, and mental health status, and shall receive a\n\ncontinuous active treatment program, which includes aggressive,\n\nconsistent implementation of a program of specialized and generic\n\ntraining, treatment, health services and related services. The\n\nevaluation and assessment shall be completed within thirty (30) days\n\nof admission to the Greer Center Facility.\n\nG. After the evaluation and assessment by the Greer Center\n\nFacility, staff shall present the referral packet of the individual\n\nand their findings to the Greer Center Admissions Committee with a\n\nrecommendation for continued admission or alternate treatment.\n\n1. The Greer Center Admissions Committee shall:\n\na. make decisions regarding continued admission, and\n\nb. notify the Department of Human Services, and the\n\nreferring agency in writing, stating specifically the\n\ndecisions of the Committee regarding admission,\n\nincluding specific reasons for denial of admission.\n\n2. If admission of an applicant is not continued at the Greer\n\nCenter Facility after undergoing the evaluation process, the\n\nreferring agency shall reimburse the Department of Human Services\n\nfor the number of bed days used at the Medicaid rate for that unit.\n\nIf admission of an applicant is continued, the applicant shall be\n\ncertified for Medicaid reimbursement from the initial date of\n\nadmission.\n\nH. Individuals who have been admitted and served by the Greer\n\nCenter Facility shall be eligible for readmission services on the\n\nsame basis as an individual initially seeking services.\n\nI. In addition to other discharge procedures and requirements\n\nprovided by law, the interdisciplinary team of the Greer Center\n\nFacility shall have recommended discharge based upon a determination\n\nthat the mental or physical condition of the individual prevents the\n\nindividual from receiving appropriate services at the Greer Center\n\nFacility or that the individual has made progress in behavioral and\n\nemotional habilitation goals such that the individual no longer\n\nrequires the specialized resources at the Greer Center Facility, and\n\nmay function in a less restrictive setting.\n\nJ. A referral to discharge is made to the Greer Center\n\nDischarge Committee, and the final decision to discharge is made by\n\nthe Greer Center Discharge Committee.\n\nK. The Greer Center Discharge Committee shall:\n\n1. Consist of a representative from the Greer Center Facility\n\nand representatives selected by the Director of the Department of\n\nHuman Services; and\n\n2. Review the discharge referral and related materials to\n\nensure that the individual can safely reside in a less restrictive\n\nsetting with appropriate supports.\nthe Greer Center Discharge Committee.\n\nK. The Greer Center Discharge Committee shall:\n\n1. Consist of a representative from the Greer Center Facility\n\nand representatives selected by the Director of the Department of\n\nHuman Services; and\n\n2. Review the discharge referral and related materials to\n\nensure that the individual can safely reside in a less restrictive\n\nsetting with appropriate supports.\n\nL.
