In order to serve as a gestational carrier under a\n\ngestational agreement, the gestational carrier must:\n\n1. Be at least twenty-one (21) years of age at the time she\n\nenters into the gestational agreement;\n\n2. Have given birth to at least one child;\n\n3. Have been a resident of Oklahoma for at least ninety (90)\n\nconsecutive days immediately preceding the date she enters into the\n\ngestational agreement, unless one or more intended parent has been a\n\nresident of Oklahoma for at least ninety (90) consecutive days\n\nimmediately preceding the date the gestational carrier enters into\n\nthe agreement;\n\n4. Have completed a physical medical evaluation relating to the\n\nanticipated pregnancy; and\n\n5. Have completed a mental health consultation.\n\nB.
