The Commission on Children and Youth shall establish the\n\nproposal submission procedures and criteria and shall promulgate\n\nrules as necessary for the implementation of the Community Youth\n\nDevelopment Act.\n\nB. In order to be eligible for a contract pursuant to the\n\nCommunity Youth Development Act the proposal, at minimum, shall:\n\n1. Meet the eligibility criteria established by the Commission;\n\n2. Be a joint proposal made by a school district, neighborhood\n\norganization, municipality or county and one or more agencies or\n\norganizations within the children and youth service system. If a\n\nschool district is not a joint participant in the proposal, the\n\nproposal shall document and describe the active participation in and\n\nsupport of the local school district in the program and activities\n\nfor which the proposal is submitted;\n\n3. Be for programs and activities for children not less than\n\nsix (6) years of age or more than nineteen (19) years of age, or in\n\ngrades one through twelve, whichever is applicable;\n\n4. Describe the respective roles and responsibilities for the\n\nadministration and operation of the program and activities,\n\nincluding but not limited to the designation of the entity\n\nresponsible for the receipt and expenditure of any funds awarded\n\npursuant to the Community Youth Development Act;\n\n5. Include a match for the costs of the proposed program from\n\nthe local school or other entity submitting the proposal. The match\n\nmay be monetary or may be an in-kind match;\n\n6. Specifically identify the area within a school district or\n\ncommunity or the neighborhood where the programs and activities will\n\nbe implemented;\n\n7. Describe how the program will coordinate and cooperate with\n\nprograms and services administered by the Department of Human\n\nServices, the State Department of Education, and other state or\n\nlocal agencies, agencies within the children and youth service\n\nsystem and courts and law enforcement, as appropriate for the\n\nproposed program;\n\n8. Provide the program and activities on site in a school,\n\ncommunity center, or similar location within the identified area of\n\nthe school district or community; and\n\n9. Include face-to-face contact with the parents, guardians or\n\ncustodians of youth participating in the program and visits to the\n\nhomes of such youth as an integral part of the programs and\n\nactivities for which the proposal is submitted.\n\nC. The Commission on Children and Youth shall evaluate at least\n\nannually each entity which receives a contract pursuant to the\n\nCommunity Youth Development Act. The evaluation report shall\n\ndocument the extent to which the program objectives have been met,\n\nas well as other information deemed necessary or appropriate by the\n\nCommission.
