Stat. tit. 10, § 10-7503-2.5","heading":"Acknowledgment of consent to adoption or permanent","body":"relinquishment before judge - Affidavit of nondisclosure.\n\nA. At the time that a written consent to adoption or permanent\n\nrelinquishment is acknowledged by a biological parent before a judge\n\nof a court of this state, the judge shall advise the biological\n\nparent:\n\n1. That an adult adopted person born in the State of Oklahoma\n\nwhose decree of adoption is finalized after November 1, 1997, may\n\nobtain such person's original certificate of birth;\n\n2. That, if affidavits of nondisclosure have been filed by both\n\nbiological parents and have not been revoked by either biological\n\nparent at the time that the request for the original birth\n\ncertificate is made by the adult adopted person, the original birth\n\ncertificate will not be released to the adult adopted person; and\n\n3. That if an unrevoked affidavit of nondisclosure by only one\n\nbiological parent is on file with the State Registrar of Vital\n\nStatistics at the time that the request for the original birth\n\ncertificate is made by the adult adopted person, identifying\n\ninformation regarding the parent who filed the unrevoked affidavit\n\nof nondisclosure will be deleted from the original birth certificate\n\nbefore it is provided to the adult adopted person. The identity of\n\nthe parent who does not have an unrevoked affidavit of nondisclosure\n\non file, if it is contained in the original birth certificate, will\n\nbe disclosed.\n\nB. 1. The judge shall ascertain whether the biological parent\n\nwishes to execute an affidavit of nondisclosure. If so, an\n\naffidavit of nondisclosure form shall be made available to the\n\nbiological parent by the court and may be executed in the presence\n\nof the judge at the time the written consent to adoption or\n\nrelinquishment for adoption is acknowledged.\n\n2. An affidavit of nondisclosure signed at the time a consent\n\nor relinquishment is acknowledged shall be filed in the adoption\n\naction with the consent or relinquishment for adoption.\n\n3. Affidavit of nondisclosure forms shall also be available in\n\neach district court clerk's office and may be executed and filed by\n\na biological parent in the court in which an adoption action is\n\npending.\n\n4. An affidavit of nondisclosure may be filed after a final\n\ndecree of adoption has been entered.\n\nC. A biological parent who has executed an affidavit of\n\nnondisclosure may revoke the affidavit of nondisclosure at any time\n\nby filing a revocation with the State Registrar of Vital Statistics.\n\nUpon receipt of a revocation of an affidavit of nondisclosure, the\n\nState Registrar shall attach the revocation to the affidavit of\n\nnondisclosure and file it with the original certificate of birth and\n\nother records of the adoption.\n\nD.
