The Disclosure Statement of Adoption-related Costs and\n\nExpenditures required by Section 7505-3.2 of Title 10 of the\n\nOklahoma Statutes shall be in substantially the following form:\n\nDISCLOSURE STATEMENT OF ADOPTION-RELATED COSTS AND EXPENDITURES\n\nThe following is a declaration of all fees, expenses, and costs\n\ncharged or expected to be charged for the adoption:\n\n1. Retainer fees in the amount of ______________________;\n\n2. The hourly billing rate of _____________________ and the\n\nnumber of hours billed for the adoption _____________________ for a\n\ntotal amount of ____________________;\n\n3. All fees charged for preplacement or other home studies of\n\nany prospective birth parents, regardless of whether the study was\n\nperformed by a third-party ___________________;\n\n4. All costs, fees, or expenses or any other thing of value\n\npaid to or on behalf of the birth parents related to the adoption by\n\nany party other than the adoptive parents __________________________\n\n_______________________________________________________________; and\n\n5. Any other fees and expenses related to the adoption not\n\npreviously listed in this disclosure statement _____________________\n\n__________________________________________________________________.\n\nB.
