Before the final hearing on the petition for adoption, the\n\nfollowing must be filed in the proceeding when available:\n\n1. A certified copy of the birth certificate or other record of\n\nthe date and place of birth of the minor;\n\n2. Any consent, extra judicial consent, or permanent\n\nrelinquishment, with respect to the minor that has been executed,\n\nand any written verifications required by the Oklahoma Adoption Code\n\nfrom the individual before whom a consent, extra judicial consent,\n\nor permanent relinquishment was executed;\n\n3. A certified copy of any court order terminating the parental\n\nrights of the minor's parents or guardian;\n\n4. A certified copy of any existing court order or the petition\n\nin any pending proceeding concerning custody of or visitation with\n\nthe minor;\n\n5. A copy of any home study performed on the petitioners,\n\nincluding the home studies required by Sections 7505-5.1, 7505-5.2\n\nand 7505-5.3 of this title;\n\n6. In an adoptive placement in which the adoptive parents or\n\nbirth parents were not both legal residents of Oklahoma prior to the\n\ninitiation of the adoption process and the parties are not otherwise\n\nexempt from the Interstate Compact on the Placement of Children, a\n\ncopy of the approval by both the sending state and receiving state\n\npursuant to the Interstate Compact on the Placement of Children;\n\n7. A copy of any agreement with a public agency to provide a\n\nsubsidy for the benefit of a minor with a special need;\n\n8. A verified document by the Department, or child-placing\n\nagency that placed the minor for adoption, or the attorney for the\n\nadoptive parent in direct placement adoption, or the person who is\n\nplacing the minor for adoption in a direct placement adoption in\n\nwhich the adoptive parent is not represented by an attorney, stating\n\nthat the petitioner for adoption has been furnished a copy of the\n\nmedical and social history report, pursuant to Section 7504-1.2 of\n\nthis title;\n\n9. The name and address, if known, of any person who is\n\nentitled to receive notice of the proceeding for adoption;\n\n10. The affidavit of expenditures;\n\n11. A copy of the medical and social history report, as\n\nrequired by subsection D of Section 7504-1.2 of this title,\n\nincluding the initial report and all supplemental reports, if any,\n\nprepared pursuant to subsection C of Section 7504-1.2 of this title;\n\n12. Affidavits of nondisclosure, if any, signed by a biological\n\nparent;\n\n13. a. A copy of the state criminal background check,\n\nnational fingerprint-based criminal background check,\n\nif required by the provisions of the Oklahoma Adoption\n\nCode, a search of the Department of Corrections' files\n\nmaintained pursuant to the Sex Offenders Registration\n\nAct, and a search of the child abuse and neglect files\n\nmaintained for review by authorized entities by the\n\nDepartment of Human Services pursuant to the Oklahoma\n\nChild Abuse Reporting and Prevention Act, or\n\nb. If the adoptive petitioners are not legal residents of\n\nOklahoma and the sending state has comparable and\n\naccessible checks and searches as specified by\n\nsubparagraph a of this paragraph, a copy of the\n\napproval of both the sending state and receiving state\n\npursuant to the Interstate Compact on the Placement of\n\nChildren or verification that this adoptive placement\n\nis otherwise exempt from the Interstate Compact on the\n\nPlacement of Children; and\n\n14. Any such other document or information required by the\n\ncourt.\n\nB.
