The Department of Human Services shall establish a\n\ncentralized paternity registry. The purpose of the registry is to:\n\n1. Protect the parental rights of a putative father who may\n\nwish to affirmatively assume responsibility for children he may have\n\nfathered; and\n\n2. Expedite adoptions of children whose biological fathers are\n\nunwilling to assume responsibility for their children by registering\n\nwith the registry or otherwise acknowledging their children.\n\nB. The father or putative father of a child born out of wedlock\n\nmay file:\n\n1. A notice of desire to receive notification of an adoption\n\nproceeding concerning the minor pursuant to Section 7503-3.1 of this\n\ntitle;\n\n2. A notice of intent to claim paternity of the child pursuant\n\nto this section or Section 7503-3.1 of this title;\n\n3. An instrument acknowledging paternity of the child as\n\nprovided in Section 7503-3.1 of this title, or this section, and\n\nSection 1-311.3 of Title 63 of the Oklahoma Statutes;\n\n4. A waiver of interest pursuant to Section 7503-3.1 of this\n\ntitle; or\n\n5. Any other claim for acknowledging or denial of paternity\n\nauthorized by law.\n\nC. The paternity registry shall also be available to any person\n\nwho:\n\n1. Has been adjudicated by a court of another state or\n\nterritory of the United States to be the father of a minor by filing\n\na certified copy of the court order with the registry; or\n\n2. Has been adjudicated by a court of this state to be the\n\nfather of a minor born out of wedlock.\n\nD. The Department shall maintain the following information in\n\nthe registry:\n\n1. The putative father's:\n\na. name,\n\nb. address at which the putative father may be served\n\nwith notice of an adoption,\n\nc. Social Security number,\n\nd. date of birth, and\n\ne. tribal affiliation, if any;\n\n2. The mother's:\n\na. name, including all other names known to the putative\n\nfather that the mother uses, and\n\nb. address, Social Security number, and date of birth, if\n\nknown;\n\n3. The minor's name, date and place of birth, if known, or the\n\nprobable month and year of the expected birth of the minor;\n\n4. The date that the Department receives a putative father's\n\nregistration;\n\n5. The:\n\na. name of an attorney or child-placing agency that\n\nrequests the Department to search the registry to\n\ndetermine whether a putative father is registered in\n\nrelation to a mother whose minor is or may be the\n\nsubject of an adoption, and\n\nb. date that the attorney or agency submits a request as\n\nprovided under this paragraph;\n\n6. If the registration is based upon an adjudication by a court\n\nof this or any other state, the case number, court, date of order,\n\njudgment or decree, and a copy of the decree; and\n\n7. Any other information that the Department determines is\n\nnecessary to access the information in the registry.\n\nE. The Department shall store the registry's data in a manner\n\nso that the data is accessible under the following:\n\n1. The putative father's name;\n\n2. The mother's name; and\n\n3. The minor's name, if known.\n\nF. A putative father who registers under this section shall\n\nprovide to the Department:\n\n1. The putative father's:\n\na. name,\n\nb. address at which the putative father may be served\n\nwith notice of an adoption,\n\nc. Social Security number,\n\nd. date of birth, and\n\ne. tribal affiliation, if any;\n\n2. The mother's name, including all other names known to the\n\nputative father that the mother uses;\n\n3. If the registration is based upon an adjudication by a court\n\nof this or any other state, the case number, court, date of order,\n\njudgment or decree, and a copy of the decree; and\n\n4. Any other information described under subsection D of this\n\nsection that is known to the putative father.\n\nG. 1. A person filing a notice of desire to receive\n\nnotification of an adoption proceeding concerning the minor, a\n\nnotice of intent to claim paternity of a minor or an acknowledgment\ny other state, the case number, court, date of order,\n\njudgment or decree, and a copy of the decree; and\n\n4. Any other information described under subsection D of this\n\nsection that is known to the putative father.\n\nG. 1. A person filing a notice of desire to receive\n\nnotification of an adoption proceeding concerning the minor, a\n\nnotice of intent to claim paternity of a minor or an acknowledgment\n\nof paternity shall include therein his current address and shall\n\nnotify the registry of any change of address pursuant to procedures\n\nprescribed by rules of the Department.\n\n2. If a putative father does not have an address where the\n\nputative father is able to receive notice of an adoption, the\n\nputative father may designate another person as an agent for the\n\npurpose of receiving notice of adoption. The putative father must\n\nprovide the Department with the agent's name and the address at\n\nwhich the agent may be served.\n\n3. Service of notice upon the agent constitutes service of\n\nnotice upon the putative father. If the agent cannot be served at\n\nthe address provided by the putative father, as provided in this\n\nsubsection, and if the putative father cannot be served because his\n\nwhereabouts are unknown, the putative father can be served by\n\npublication pursuant to paragraph 3 of subsection B of Section 7505-\n\n2.1 or paragraph 3 of subsection C of Section 7505-4.1 of this\n\ntitle.\n\nH. An unrevoked notice of intent to claim paternity of a minor\n\nor an instrument acknowledging paternity may be introduced in\n\nevidence by any party in any proceeding in which such fact may be\n\nrelevant.\n\nI. The Department, upon request, shall provide the names and\n\naddresses of persons listed with the registry to any court or\n\nauthorized agency, and such other persons deemed necessary to\n\nreceive such information by the Department. The information shall\n\nnot be divulged to any other person except upon order of a court for\n\ngood cause shown.\n\nJ. The Department shall:\n\n1. Provide the forms necessary for filing with the paternity\n\nregistry established by this section and shall make said forms\n\navailable to any father or putative father of a minor born out of\n\nwedlock who wishes to file with the registry; and\n\n2.
