Stat. tit. 10, § 10-7700-302","heading":"Requirements of acknowledgment – Void and voidable","body":"acknowledgment.\n\nA. An acknowledgment of paternity shall:\n\n1. Be in a record and on the form prescribed by the Department\n\nof Human Services pursuant to Section 20 of this act;\n\n2. Be signed, or otherwise authenticated, under penalty of\n\nperjury by the mother and by the man seeking to establish his\n\npaternity;\n\n3. State that the child whose paternity is being acknowledged:\n\na. does not have a presumed father, or has a presumed\n\nfather whose full name is stated, and\n\nb. does not have another acknowledged or adjudicated\n\nfather;\n\n4. State whether there has been genetic testing and, if so,\n\nthat the acknowledging man’s claim of paternity is consistent with\n\nthe results of the testing; and\n\n5. State that the signatories understand that the\n\nacknowledgment is the equivalent of a judicial adjudication of\n\npaternity of the child and that a challenge to the acknowledgment is\n\npermitted only under limited circumstances and is barred after two\n\n(2) years.\n\nB. An acknowledgment of paternity shall be void if it:\n\n1. States that another man is a presumed father, unless a\n\ndenial of paternity signed or otherwise authenticated by the\n\npresumed father is filed with the State Department of Health,\n\nDivision of Vital Records; or\n\n2. States that another man is an acknowledged or adjudicated\n\nfather.\n\nC. An acknowledgment of paternity is voidable if it falsely\n\ndenies the existence of a presumed, acknowledged, or adjudicated\n\nfather of the child.\n\nD.
