A report of genetic testing shall be in a record and signed\n\nunder penalty of perjury by a designee of the testing laboratory. A\n\nreport made under the requirements of this Article will be admitted\n\nas evidence of paternity without the need for foundation testimony\n\nor other proof of authenticity or accuracy.\n\nB. Documentation from the testing laboratory of the following\n\ninformation is sufficient to establish a reliable chain of custody\n\nthat allows the results of genetic testing to be admissible without\n\ntestimony:\n\n1. The names and photographs of the individuals whose specimens\n\nhave been taken;\n\n2. The names of the individuals who collected the specimens;\n\n3. The places and dates the specimens were collected;\n\n4. The names of the individuals who received the specimens in\n\nthe testing laboratory; and\n\n5.
