Either parent or the court-appointed legal guardian of a\n\nchild may authorize, in writing, any adult person into whose care\n\nthe minor has been entrusted to consent to any:\n\na. x-ray examination,\n\nb. anesthetic,\n\nc. medical or surgical diagnosis or treatment,\n\nd. hospital care, or\n\ne. immunization, blood tests, examinations, Guidance\n\nServices, and Early Intervention Services provided by\n\na city or county Department of Health,\n\nto be rendered to said minor under the general or special\n\nsupervision and upon the advice of a physician and surgeon licensed\n\nunder the laws of the State of Oklahoma, or to consent to an x-ray\n\nexamination, anesthetic, dental or surgical diagnosis or treatment\n\nand hospital care to be rendered to said minor by a dentist licensed\n\nunder the laws of the State of Oklahoma.\n\n2. If any parent or other person falsely represents in writing\n\nthat such parent or other person has legal custody or legal\n\nguardianship of the minor child, or if any adult falsely represents\n\nthat the written authorization provided for in this subsection is\n\nvalid, and a health professional provides health services or care as\n\nprovided by this section in good faith upon such misrepresentation,\n\nthe health professional shall incur no liability except for\n\nnegligence or intentional harm.\n\nB. Either parent, if both parents have legal custody, or the\n\nparent or person having legal custody or the legal guardian of a\n\nminor may authorize, in writing, pursuant to the provisions of\n\nSection 1-116.2 of Title 70 of the Oklahoma Statutes a school or\n\ncounty nurse or in the absence of such nurse, a school administrator\n\nor designated school employees to administer:\n\n1. A nonprescription medicine; and\n\n2.
