A petition in a proceeding alleging a child to be\n\ndeprived may be filed by the district attorney to determine if\n\nfurther action is necessary. The proceeding shall be entitled \"In\n\nthe matter of ____________, an alleged deprived child\".\n\n2. The petition shall be verified and may be upon information\n\nand belief. The petition shall set forth:\n\na. with particularity, facts which bring the child within\n\nthe purview of this chapter,\n\nb. the name, date of birth, and residence of the child,\n\nc. the names and residences of the child's parents,\n\nd. the name and residence of the child's legal guardian,\n\nif there is one,\n\ne. the name and residence of the person or persons having\n\ncustody or control of the child,\n\nf. the name and residence of the nearest known relative,\n\nif no parent, legal guardian or custodian of the child\n\ncan be found, and\n\ng. the relief requested including, but not limited to, or\n\nwhere applicable:\n\n(1) an adjudication that the child is deprived,\n\n(2) a termination of parental rights,\n\n(3) the entry of an order for child support, and\n\n(4) a judicial determination of the child’s\n\npaternity.\n\n3. If any of the facts herein required are not known by the\n\npetitioner, the petition shall so state, along with the reasons why\n\nsuch facts are not known to petitioner.\n\nB. A petition alleging a child to be a minor in need of\n\ntreatment shall be filed by a district attorney pursuant to the\n\nInpatient Mental Health and Substance Abuse Treatment of Minors Act\n\nas provided for in Sections 5-501 through 5-513 of Title 43A of the\n\nOklahoma Statutes.\n\nC. A copy of the petition alleging a child to be deprived shall\n\nbe attached to and delivered with the summons.\n\nD.
