Upon the filing of the petition, the court shall\n\nschedule a hearing and shall issue a summons requiring the parents,\n\nlegal guardian, custodian, the child if the child is twelve (12) or\n\nmore years of age, and any other persons the court determines to be\n\nproper or necessary parties to the proceedings to appear personally\n\nbefore the court at the date, time, and place stated in the summons.\n\nThe court may endorse upon the summons an order directing the\n\nparent, guardian, custodian, or other person having the physical\n\ncustody or control of the child to bring the child to the hearing.\n\n2. The summons shall be attached to a copy of the petition and\n\nshall advise the parties of the right to counsel, including the\n\nright of the child’s parent or legal guardian to court-appointed\n\ncounsel if indigent.\n\n3. The summons shall state the relief requested, including\n\nnotice that child support may be ordered or modified and that the\n\nchild’s paternity, if at issue, may be established.\n\n4. The summons shall also contain, in type at least as large as\n\nthe balance of the document, the following or substantially similar\n\nlanguage: \"FAILURE TO RESPOND TO THIS SUMMONS OR TO APPEAR AT THIS\n\nHEARING CONSTITUTES CONSENT TO THE ADJUDICATION OF THIS CHILD (OR\n\nTHESE CHILDREN) AS DEPRIVED CHILDREN AND MAY ULTIMATELY RESULT IN\n\nLOSS OF CUSTODY OF THIS CHILD OR THE TERMINATION OF PARENTAL RIGHTS\n\nTO THIS CHILD.\"\n\nB. A party other than the child may waive service of summons in\n\nwriting or by voluntary appearance at the hearing. A child’s\n\ncounsel may waive service of summons on the child’s behalf.\n\nC.
