The Department of Human Services or the agency having\n\nsupervision of the case or, if the child has been removed from the\n\ncustody of the child's parents, the Department or the agency or\n\nchild-placing agency having custody of the child shall cause to be\n\nprepared for each review hearing required herein a written report\n\nconcerning each child who is the subject of review.\n\nB. The report shall include, but not be limited to:\n\n1. A summary of the physical, mental, and emotional condition\n\nof the child, the conditions existing in the out-of-home placement\n\nwhere the child has been placed, and the adjustment of the child\n\nthereto;\n\n2. A report on the progress of the child in school and, if the\n\nchild has been placed outside the child's home, the visitation\n\nexercised by the parents of the child or other persons authorized by\n\nthe court;\n\n3. Services being provided to a child sixteen (16) years of age\n\nor older to assist in the transition from out-of-home care or other\n\ncommunity placement to independent living;\n\n4. When the Department is responsible for supervision of the\n\nchild or is the legal custodian of the child, a description of:\n\na. progress on the part of the parent or parents to\n\ncorrect the conditions which caused the child to be\n\nadjudicated deprived,\n\nb. changes that still need to occur and the specific\n\nactions the parents would take to make the changes,\n\nand\n\nc. services and assistance that have been offered or\n\nprovided to the parents since the previous hearing and\n\nthe services which are needed in the future;\n\n5. A description of the placements of the child by number and\n\ntype with dates of entry and exit, reasons for the placement or\n\nchange in placement, and a statement about the success or lack of\n\nsuccess of each placement;\n\n6. The efforts of the Department to locate the parents and\n\ninvolve them in the planning for the child if the parents are not\n\ncurrently communicating with the Department;\n\n7. Compliance by the Department, as applicable, and the parent\n\nwith the court's orders concerning the individualized service plans,\n\nprevious court orders, and the Department recommendations;\n\n8. Whether the current placement is appropriate for the child,\n\nits distance from the home of the child, and whether it is the least\n\nrestrictive, most family-like placement available;\n\n9. A proposed timetable for the return of the child to the home\n\nor other permanent placement; and\n\n10. Specific recommendations, giving reasons whether:\n\na. trial reunification should be approved by the court,\n\nb. trial reunification should be continued to a date\n\ncertain as specified by the court,\n\nc. the child should remain in or be placed outside of the\n\nhome of the parent or legal guardian of the child, or\n\nd. the child should remain in the current placement when\n\nthe permanency plan is other than reunification with\n\nthe parent or legal guardian of the child.\n\nC. The attorney representing a child, the foster parents of the\n\nchild and the guardian ad litem of a child, if any, whose case is\n\nbeing reviewed may submit a report to the court for presentation at\n\nthe review hearing to assist the court in reviewing the placement or\n\nstatus of the child.
