Stat. tit. 10A, § 10A-1-7-104","heading":"Information to accompany child placed outside child's","body":"home – Passport Program.\n\nA. The court shall ensure that the following information\n\naccompanies any deprived child placed outside the child's home as\n\nsoon as the information becomes available:\n\n1. Demographic information;\n\n2. Strengths, needs and general behavior of the child;\n\n3. Circumstances which necessitated placement;\n\n4. Type of custody and previous placement;\n\n5. Pertinent family information including, but not limited to,\n\nthe names of family members who are and who are not, by court order,\n\nallowed to visit the child and the child's relationship to the\n\nfamily which may affect placement;\n\n6. Known and important life experiences and relationships which\n\nmay significantly affect the child's feelings, behavior, attitudes\n\nor adjustment;\n\n7. Whether the child has third-party insurance coverage which\n\nmay be available to the child;\n\n8. Education history to include present grade placement, last\n\nschool attended, and special strengths and weaknesses. The\n\nDepartment of Human Services shall also assist the foster parents in\n\ngetting the child admitted into school and obtaining the child's\n\nschool records; and\n\n9. Known or available medical history including, but not\n\nlimited to:\n\na. allergies,\n\nb. immunizations,\n\nc. childhood diseases,\n\nd. physical handicaps,\n\ne. psycho-social information, and\n\nf. the name of the child's last doctor, if known.\n\nB. When the Department places a child in out-of-home care, the\n\nDepartment shall provide the placement providers with sufficient\n\nmedical information to enable the placement providers to care for\n\nthe child safely and appropriately. Such medical information shall\n\ninclude, but not be limited to:\n\n1. Any medical or psychological conditions;\n\n2. Diseases, illnesses, accidents, allergies, and congenital\n\ndefects;\n\n3. The child's Medicaid card or information on any other third-\n\nparty insurer, if any; and\n\n4. Immunization history.\n\nC. 1. The Department of Human Services shall establish a\n\nPassport Program for children in the custody of the Department.\n\n2. The Program shall provide for a Passport, which shall be a\n\ncompilation of the significant information provided for in\n\nsubsections A and B of this section for each child, in particular,\n\neducation and physical and behavioral health records.\n\n3. In furtherance of the purposes of this section, the Oklahoma\n\nHealth Care Authority, the Department of Education, and the\n\nDepartment of Mental Health and Substance Abuse Services shall\n\ncooperate with the Department to establish the Passport Program.\n\n4. The Passport shall accompany each child to wherever the\n\nchild resides so long as the child is in the custody of the\n\nDepartment and the Department shall:\n\na. work with public and private partners to gain access\n\nto the information listed in subsections A and B of\n\nthis section,\n\nb. provide for a secure database in which to store the\n\ninformation, and\n\nc. consult with the Oklahoma Health Care Authority to\n\nconvert Medicaid claims data to a usable format and to\n\nadd it from other data sources in order to provide\n\nfoster families more information about the history and\n\nneeds of the child.\n\n5. For the purposes of Section 1 of this act, the secure\n\ndatabase created to store Passport information shall be made\n\navailable to the Office of Juvenile Affairs.
