Stat. tit. 11, § 11-4-101","heading":"Petition for incorporation of city - Contents","body":"A petition for incorporation of a city shall be filed with the\n\nboard of county commissioners of the county in which the proposed\n\ncity is located. The petition shall:\n\n1. Be in writing; and\n\n2. Be signed by at least thirty-five percent (35%) of the\n\nregistered voters residing in the proposed city, as shown by the\n\npreceding general election.\n\nEach petition shall be on a separate sheet and shall be\n\nauthenticated by the affidavit of at least one credible witness that\n\nthe signatures are genuine and the signers of the petition are\n\nregistered voters of the proposed city. The petition shall include:\n\n1. The name of the proposed city;\n\n2. The survey and plat of the proposed city;\n\n3. The resident population according to the latest federal\n\ncensus or other census recognized by the laws of Oklahoma, which\n\npopulation must be one thousand (1,000) inhabitants or more;\n\n4. The number and boundaries of the proposed city wards;\n\n5. A designation of the statutory form of city government that\n\nis proposed for the city when it becomes incorporated; and\n\n6.
