Garnishment proceedings, whether prejudgment or\n\npostjudgment, shall be commenced by the filing of an affidavit, on a\n\nform prescribed by the Oklahoma Bar Association. The Administrative\n\nOffice of the Courts shall provide public access to the affidavit by\n\nproviding a link to the form on the Oklahoma State Courts Network\n\n(OSCN) website, stating:\n\n1. The name(s) of the plaintiff(s);\n\n2. The name(s) of the defendant(s);\n\n3. In the case of prejudgment garnishments, the amount of the\n\nplaintiff's original claim against the defendant or defendants over\n\nand above all offsets;\n\n4. In the case of postjudgment garnishments, the amount of the\n\ninterest-bearing balance;\n\n5. In the case of postjudgment garnishments, the rate and the\n\ndate the interest begins to accrue; and\n\n6. That the plaintiff verily believes that some person, naming\n\nhim or her, whether within or without the county, is indebted to or\n\nhas property in his or her possession or under his or her control\n\nbelonging to the defendant, or either or any of the defendants, in\n\nthe action or execution and that the indebtedness or property is, to\n\nthe best of the knowledge and belief of the person making such\n\naffidavit, not by law exempt from seizure or sale upon execution.\n\nB. The affidavit may be filed by the plaintiff or the\n\nplaintiff's attorney at or before the time of filing of a\n\ngarnishment summons.\n\nC. Only one garnishee may be embraced in any affidavit or\n\ngarnishment summons.\n\nR.L. 1910, § 4823.
