When a plaintiff requests an indigency exemption from\n\nproviding an affidavit of merit in a civil action for negligence\n\npursuant to Section 2 of this act, such person shall submit an\n\nappropriate application to the court clerk, on a form created by the\n\nAdministrative Director of the Courts, which shall state that the\n\napplication is signed under oath and under the penalty of perjury\n\nand that a false statement may be prosecuted as such.\n\nB. 1. The Supreme Court shall promulgate rules governing the\n\ndetermination of indigency for a plaintiff claiming an exemption\n\nfrom providing an affidavit of merit in a civil action for\n\nnegligence pursuant to Section 2 of this act. The initial\n\ndetermination of indigency shall be made by the Chief Judge of the\n\nJudicial District or a designee thereof, based on the plaintiff's\n\napplication and the rules provided herein.\n\n2.
