Stat. tit. 12, § 12-193","heading":"Required information for civil action to collect medical","body":"debt.\n\nA. For purposes of this section, “hospital price transparency\n\nlaws” means:\n\n1. Section 2718(e) of the Public Health Service Act, 42 U.S.C.,\n\nSection 300gg-18, as amended, and rules adopted by the United States\n\nDepartment of Health and Human Services implementing Section\n\n2718(e); and\n\n2. The Transparency in Health Care Prices Act, Section 1-725.1\n\net seq. of Title 63 of the Oklahoma Statutes.\n\nB. A creditor or debt collector or collection agency operating\n\non behalf of a creditor that files a civil action for recovery of a\n\nmedical debt shall attach to the petition or applicable form:\n\n1. A copy of redacted itemization of the charges that are the\n\nbasis for the medical debt; and\n\n2. Proof of compliance with hospital price transparency laws.\n\nC. Prior to entry of a default judgment against a consumer in a\n\ncivil action on a medical debt, in addition to compliance with the\n\napplicable rules of the district court for entry of a default\n\njudgment, the plaintiff shall file with the court evidence that\n\nestablishes the amount and nature of the medical debt and includes:\n\n1. The original account number at charge-off;\n\n2. The original creditor at charge-off;\n\n3. The amount due at charge-off or, if the balance has not been\n\ncharged off, an itemization of the amount claimed to be owed\n\nincluding the principal, interest, fees, and other charges or\n\nreductions from payment made or other credits;\n\n4. An itemization of post charge-off additions, if any;\n\n5. The date of the last payment, if applicable, or the date of\n\nthe last transaction; and\n\n6.
