Stat. tit. 12, § 12-3240","heading":"Disclosure statement","body":"Not less than three (3) days prior to the date on which a payee\n\nsigns a transfer agreement, the transferee shall provide to the\n\npayee a separate disclosure statement, in bold type no smaller than\n\nfourteen (14) point, to include the following:\n\n1. The amounts and due dates of the structured settlement\n\npayments to be transferred;\n\n2. The aggregate amount of the payments;\n\n3. The discounted present value of the payments to be\n\ntransferred, which shall be identified as the “calculation of\n\ncurrent value of the transferred structured settlement payments\n\nunder federal standards for valuing annuities”, and the amount of\n\nthe applicable federal rate used in calculating such discounted\n\npresent value;\n\n4. The gross advance amount;\n\n5. An itemized listing of all applicable transfer expenses,\n\nother than attorneys’ fees and related disbursements payable in\n\nconnection with the transferee’s application for approval of the\n\ntransfer, and the transferee’s best estimate of the amount of any\n\nsuch fees and disbursements;\n\n6. The net advance amount;\n\n7. The amount of any penalties or liquidated damages payable by\n\nthe payee in the event of any breach of the transfer agreement by\n\nthe payee; and\n\n8.
