Except as provided by subsection B of this section, whenever\n\nunder any law of this state or under any rule, order or requirement\n\nmade pursuant to the law of this state, any matter is required or\n\npermitted to be supported, evidenced, established or proved by the\n\nsworn statement, declaration, verification, certificate, oath or\n\naffidavit, in writing of the person making the same, the matter may\n\nwith like force and effect be supported, evidenced, established or\n\nproved by the unsworn statement in writing of the person made and\n\nsigned under penalty of perjury setting forth the date and place of\n\nexecution and that it is made under the laws of this state. The\n\nstatement may be substantially in the following form:\n\n\"I state under penalty of perjury under the laws of Oklahoma\n\nthat the foregoing is true and correct.\n\n______________________________ _________________________________\n\n(Date and Place) (Signature)\"\n\nThe signed statement shall constitute a legally binding assertion\n\nthat the contents of the statement to which it refers are true.\n\nThis section shall not affect any requirement for acknowledgment of\n\nan instrument affecting real property.\n\nB. The provisions of subsection A of this section shall not\n\napply to:\n\n1. A deposition;\n\n2. Any oath of office;\n\n3. An oath required to be taken before a specified official\n\nother than a notary public; or\n\n4.
