Stat. tit. 13, § 13-177.3","heading":"Application for order or extension of order","body":"An officer, attorney or agent of any law enforcement agency, a\n\ndistrict attorney or assistant district attorney or the Attorney\n\nGeneral or Assistant Attorney General may make application for an\n\norder or an extension of an order as provided in Section 4 of this\n\nact. Such application shall be in writing, under oath or equivalent\n\naffirmation, to a court of competent jurisdiction and shall include:\n\n1. The identity of the person making the application and the\n\nidentity of the law enforcement agency conducting the investigation;\n\nand\n\n2.
