An amendment under this act must identify the owner, the\n\nreal property affected, and the document containing the unlawful\n\nrestriction. The amendment must include a conspicuous statement in\n\nsubstantially the following form:\n\n\"This amendment removes from this deed or other document\n\naffecting title to real property an unlawful restriction as defined\n\nunder the Oklahoma Uniform Unlawful Restrictions in Land Records\n\nAct. This amendment does not affect the validity or enforceability\n\nof a restriction that is not an unlawful restriction.\"\n\nB. The amendment must be executed and acknowledged in the\n\nmanner required for recordation of a document in the land records.\n\nThe amendment must be recorded with the county clerk in the county\n\nin which the document containing the unlawful restriction is\n\nrecorded.\n\nC. The amendment does not affect the validity or enforceability\n\nof any restriction that is not an unlawful restriction.\n\nD.
