A master form shall be any instrument containing forms of\n\ncovenants, conditions, obligations, powers, or other clauses not\n\nbeing acknowleged nor containing a legal description. Such\n\ninstrument shall be labeled on the first page as a master form and\n\nmay be filed and recorded in the office of the county clerk in the\n\nsame manner as provided for miscellaneous instruments relating to\n\nreal estate.\n\nB. After recording and indexing of the master form, any of the\n\nprovisions of such master form may be incorporated by reference in\n\nany subsequently recorded instrument. Provided, that, such\n\nreference shall state that the master form was recorded in the\n\ncounty in which the subsequently recorded instrument is offered for\n\nrecording, the date of recording and the book and pages where\n\nrecorded, and that a copy of such master form was furnished to the\n\npersons executing the subsequently recorded instrument. The\n\nrecording of any subsequently recorded instrument which has so\n\nincorporated by reference any of the provisions of a master form\n\nproperly recorded shall have like effect as if such provisions of\n\nthe master form so incorporated by reference had been set forth in\n\nfull in the subsequently recorded instrument.\n\nC. Whenever a subsequently recorded instrument is presented for\n\nrecording in which is set forth matters purporting to be a copy or\n\nreproduction of a master form and such matters are preceded by the\n\nwords, \"do not record or not to be recorded\", and such matters are\n\nplainly separated from such instrument in such manner that such\n\nmatter will not appear upon a photographic reproduction of any page\n\ncontaining any part of the instrument, such matter shall not be\n\nrecorded by the county clerk when the instrument is recorded.
