(year).\n\nState of Oklahoma, )\n\n)ss.\n\nCounty of _________)\n\nTo the Governor of the State of Oklahoma:\n\nI most respectfully represent that I am _____ years of age; and\n\nam now and have been for _____ years a resident of said county and\n\nstate above mentioned; that I am now residing within the precinct or\n\nward in which I now propose to vote, and have resided thereat for\n\n_____ years, and am a duly qualified elector in the precinct wherein\n\nI reside; and I do most respectfully petition Your Excellency to\n\ncause to be placed the name of ______ (city, town or place) upon the\n\ntickets to be voted at the special election to be held in ________\n\nCounty on the ____ day of _____, A.D.
