The certified organization shall prepare and distribute all\n\nnecessary ballots in advance of the referendum and election and\n\nshall cause ballots to be available at all polling places.\n\nB. The referendum provisions of the ballot shall specify a\n\nmaximum rate for the authorized assessment.\n\nC. The election provisions of the ballot may be printed only\n\nwith the names of candidates who have filed valid petitions under\n\nSection 5-60.15 of this title, but the ballot shall provide a space\n\nfor write-in votes.\n\nD. The ballot shall provide a space for the voter to certify\n\nthe volume of the voter's production of the commodity during the\n\npreceding year or other relevant production period, as designated on\n\nthe ballot.\n\nE.
