The question of law to be answered;\n\n2. The facts relevant to the question, showing fully the nature\n\nof the controversy out of which the question arose;\n\n3. A statement acknowledging that the Supreme Court or Court of\n\nCriminal Appeals of this state, acting as the receiving court, may\n\nreformulate the question; and\n\n4. The names and addresses of counsel of record and parties\n\nappearing without counsel.\n\nB.
