Stat. tit. 21, § 21-1290.18","heading":"Application form contents","body":"APPLICATION FORM CONTENTS\n\nThe application for a handgun license shall be completed upon\n\nthe sworn oath of the applicant as provided in paragraph 5 of\n\nSection 1290.12 of this title. The application form shall be\n\nprovided by the Oklahoma State Bureau of Investigation and shall\n\ncontain the following information in addition to any other\n\ninformation deemed relevant by the Bureau:\n\n1. Applicant's full legal name;\n\n2. Applicant's birth name, alias names or nicknames;\n\n3. Applicant's maiden name, if applicable;\n\n4. County of residence;\n\n5. Length of residency at the current address;\n\n6. Previous addresses for the preceding three (3) years;\n\n7. Place of birth;\n\n8. Date of birth;\n\n9. Declaration of citizenship or alien or admission number for\n\na non-United-States citizen;\n\n10. Race;\n\n11. Weight;\n\n12. Height;\n\n13. Sex;\n\n14. Color of eyes;\n\n15. Current driver license number;\n\n16. Military service number, if applicable;\n\n17. Law enforcement identification numbers, if applicable;\n\n18. Current occupation;\n\n19. Authorized type or types of pistol for which the applicant\n\nqualified as stated on the certificate of training or exemption of\n\ntraining which shall be stated as either derringer, revolver,\n\nsemiautomatic pistol, or some combination of derringer, revolver and\n\nsemiautomatic pistol and the maximum ammunition capacity of the\n\nfirearm shall be .45 caliber;\n\n20. An acknowledgment that the applicant desires a handgun\n\nlicense as a means of lawful self-defense and self-protection and\n\nfor no other intent or purpose;\n\n21. A statement that the applicant has never been convicted of\n\nany felony offense in this state, another state or pursuant to any\n\nfederal offense;\n\n22. A statement that the applicant has none of the conditions\n\nwhich would preclude the issuing of a handgun license pursuant to\n\nany of the provisions of Sections 1290.10 and 1290.11 of this title\n\nand that the applicant further meets all of the eligibility criteria\n\nrequired by Section 1290.9 of this title;\n\n23. An authorization for the Oklahoma State Bureau of\n\nInvestigation to investigate the applicant and any or all records\n\nrelating to the applicant for purposes of approving or denying a\n\nhandgun license pursuant to the provisions of the Oklahoma Self-\n\nDefense Act;\n\n24. An acknowledgment that the applicant has reviewed the\n\nFederal Bureau of Investigation Privacy Act Statement and the\n\nOklahoma Self-Defense Act and is knowledgeable about its provisions;\n\n25. A statement that the applicant is the identical person who\n\ncompleted the firearms training course for which the original\n\ntraining certificate is submitted as part of the application or a\n\nstatement that the applicant is the identical person who is exempt\n\nfrom firearms training for which the original exemption certificate\n\nis submitted as part of the application, whichever is applicable to\n\nthe applicant;\n\n26. A conspicuous warning that the application is executed upon\n\nthe sworn oath of the applicant and that any false or misleading\n\nanswer to any question or the submission of any false information or\n\ndocumentation by the applicant is punishable by criminal penalty as\n\nprovided in paragraph 5 of Section 1290.12 of this title;\n\n27. A signed verification that the contents of the application\n\nare known to the applicant and are true and correct;\n\n28. Two separate places for the original signature of the\n\napplicant;\n\n29. A place for attachment of a passport-sized photograph of\n\nthe applicant; and\n\n30.
